Our elders deserve to be home, surrounded by family and community. The Pathways Program helps elders transition from nursing homes and rehab centers back into their homes—where they belong.
The Pathways Program provides support with Medicaid/Sooner Care enrollment, medical supplies, and home modifications to ensure they can live safely and comfortably.
