Closure Notice
In observance of Juneteenth, we will be closed Friday, June 19th.
Juneteenth commemorates the emancipation of enslaved African Americans and serves as a time to reflect on the enduring values of freedom, resilience, and self-determination. We honor this important day by recognizing the strength of communities who have persevered through generations of adversity and continue to shape a brighter future for all.
Patients are encouraged to pick up prescription medications and refill any needed prescriptions before the holiday closure to avoid interruptions in care.

You will be contacted within a week to notify you of where your application is. Remember, ANY 8-12-year-old of any tribe can be signed up!
