If you’re experiencing dental pain or have an urgent dental concern, please call us as soon as possible. Our team will work with you to determine the most appropriate appointment.

APPOINTMENT INFORMATION

Please check in directly at the Dental Clinic for your appointment. You may enter through the North Entrance or use the Main Lobby and follow the signs to the Dental Clinic.

BEFORE YOUR APPOINTMENT

To help your first visit go smoothly, please remember to bring:

• Current State Photo ID/Driver’s License
• Social Security Card
• U.S. Federally Recognized Tribal Membership/Citizenship card OR Certificate Degree of Indian Blood (CDIB)
• Dental Insurance Cards (Private Insurance, Medicare or Medicaid)
• Current Proof of Residency (For patients who reside within the Kickapoo Contract Health Service boundaries, Proof of Residence includes utility bill, pay stub, lease/rental agreement or mortgage statement w/current address and current month)

For Minors:

• Parent/Guardian’s Current State Photo ID/Driver’s License
• Social Security Card
• U.S. Federally Recognized Tribal Membership/Citizenship card OR Certificate Degree of Indian Blood (CDIB)
• Vital Statistics or State Certified Birth Certificate
• Insurance Cards (Private Insurance, Medicare or Medicaid)
• Current Proof of Residency (For patients who reside within the Kickapoo Contract Health Service boundaries, Proof of Residence includes utility bill, pay stub, lease/rental agreement or mortgage statement in parent/guardian’s name w/current address and current month)
• Legal Guardianship Documentation (If applicable)

Arrive at least 30 minutes before your scheduled appointment to complete the check-in process.

Arrive on time. If an unexpected delay occurs, please call us as soon as possible. Patients who arrive late may need to be rescheduled. Please refer to our Cancellation/No-Show Policy for additional information.
